Insurance Plans

The Eye Doctors at Columbus Eye Associates have agreements with most Health Plans, Third Party Administrators and Physician Networks. If we verify your insurance benefits with your insurance company, it is not a guarantee of payment of benefits. Payment of benefits is subject to several factors, including, but not limited to, eligibility at the time of service, payment of premiums/contributions, amounts allowable for services, supporting medical documentation and other terms, conditions, limitations and exclusions set forth in your Certificate of Benefits Booklet and/or Summary Plan Description as well as the preexisting condition waiting period, if any. This is why we do not guarantee that the Eye Doctors at Columbus Eye Associates are a participating provider under your Insurance Plan. It is the patient’s responsibility to verify that the Eye Doctor and the Office Location is considered In-Network on your insurance plan to ensure that you will receive an In-Network Benefit on the services that they receive.
